Made Simple Medical's Insurance and Paperwork Policy

We do not accept health insurance of any type, nor do we normally work directly with insurance companies. If you do have health insurance and wish to use it for services rendered or to fill orders from our providers it is your responsibility to determine how to do so and what information you would need. If any paperwork needs to be completed or provider time is needed otherwise it will either have to be during the time of your visit or you will be charged at a rate of $25 per page or $300 per hour (billed in 5 minute increments), whichever is greater. This charge needs to be paid in full before any paperwork or provider time is given. This includes, but is not limited to FMLA paperwork, disability paperwork, Prior Authorizations, School forms, Doctor's Notes, expert opinions, etc.  We can provide an itemized Superbill for services rendered free of charge upon request at time of service.
